05 Kidney - Heart RAAS 혈압조절, 염분이 부족하면 혈압조절에 스트레스 상태(스테로이드-혈압상승)가 된다.

  RAAS 란 Renin-Angiotensin-Aldosteron-System 이다. 여러 호르몬이 단계적으로 작용하는 시스템이다. RAAS는 혈압이 낮을 때 작용하는 시스템이다. 위에 그림 혈관을 참고해보자. 위에 빨간색으로 혈관 하나가 있는데, 신장(kidney)에 존재하는 혈관이다. 수입소동맥과 수출소동맥이 있는데, 상대적으로 수입소동맥의 혈관 둘레가 더 크다.  어쨌든, 혈관에 존재하는 방사체세포가 혈압을 체크하는데, 여기서 혈압이 낮다고 판단이 되면, 신장에 자극을 주어  레닌이라는 물질을 만들어낸다. 이 레닌은 비활성 상태로 존재하고 있는 angiotensinogen이라는 물질을 angiotension1 으로  활성화시킨다. angiotension1은 신장소동맥을 수축시켜 일시적으로 대동맥압을 높인다.  우리 몸의 순환계는 체순환, 폐순환, 태아순환, 림프순환, 문맥순환 등등이 존재한다. 많은 혈관을 지나다니면서 몸에  영양분을 공급해주고 노폐물을 제거시켜주는데, 체순환을 할때, 혈관이 여러 갈래로 퍼져나가게 된다. 이때 신장으로 들어가는 혈관을  신장소동맥이라고 하는데, 이 신장소동맥을  잠궈버리면(수축) 신장으로 피가 그만큼 못 가게 되고, 대동맥에 그만큼의 피가 존재하게  되므로, 혈압이 높아진다는 뜻이다.  여기서 신장소동맥과 수출소동맥은 다른 혈관임을 인지하자. 그리고 angiotension1은 angiotensin - converting - enzyme(ACE)에 작용하여 angiotension2가 만들어지게 된다.  이 angiotension2는 부신피질(adrenal gland cortex)를 자극하게 되는데 부신피질에서 나오는 호르몬의 성분은 모두 steroid 성분이다. 아무튼 부신피질에서 mineralocorticoid를 분비하게 된다. 미네랄로코르티코이드는 알도스테론이라고도 부른다. 알도스테론 호르몬의 target site는 집합관...
